Add a consumer guide

A consumer adopting the 5.28.2 to 5.45.0 series had to reconstruct it
from twenty-three changelog entries and a dozen cross-session messages.
This is the reference that should have existed.

Covers the nine RNA-evidence columns and what each derivation means, the
four paths to a ProteinFragment and the shape they share, the DSL's
ambiguity resolution and per-peptide alleles, the shared helpers that
exist precisely so consumers stop reimplementing them, the reader escape
hatches, and a floor-by-feature table. Also records what 5.45.0 removed
with no shim, and why: five pieces of API that existed to work around
carrying isovar's fragment counts under a name for reads.

Every code example was executed against the shipped package rather than
written from memory.
