ir: CFG mid-end; drop unreachable blocks and jump-to-next (#251)

* ir: CFG container as a tested library

Procedure::body stays linear. buildCfg/flattenCfg split implicit
blocks; validateProcedureBody is validateCfg(buildCfg(body)).
No production CFG pass yet.

* ir: CFG mid-end; drop unreachable blocks and jump-to-next

applyCfgPasses builds a CFG, deletes unreachable blocks, folds
GOTO to the next label, and flattens. The linear jump-to-next
peephole is gone. Unreachable elim is always on until -O1 exists.
