perf(size): relocate fold ledger + probe behind the optimistic hook

heldTruthNodes and transitionHoldsOptimism move from scheduler.ts into
store/next/optimistic.ts (their only consumer), and read()'s latest()/
authoritative-read exemptions move inside _heldTruthMasked, which now
takes the observer. Core floor and every non-optimistic bundle shed the
bytes (core 7.94 -> 7.93, hydrating no-store 17.59 -> 17.54, rowProof
16.93 -> 16.90); the store-family scenario is flat — the bytes moved
into the module it already retains. Caps re-pinned to measurements.
