feat: add enterprise configuration system, CI/CD pipeline controls, and overlap filtering

- Introduce a fluent `Config` builder pattern loaded from `phpcpd.config.php` with a dist template
- Add `--quiet` option to suppress non-essential CLI output for CI/CD environments (fixes #205)
- Implement CI/CD exit code controls via `--max-percentage` and `--ignore-violations-on-exit` options (fixes #190)
- Add an overlap filter to automatically prevent duplicate reporting of intersecting or nested clone ranges
