chore: harden coverage reporting

Reporting forwards the workflow token to a third party, and the workflow
pinned no permissions of its own, so what left the machine was whatever the
organisation default happened to be at the time. Capping it in the workflow
means a settings change elsewhere cannot silently widen it.

A badge cannot fail, so on its own it records regressions rather than
preventing them. The threshold gives the number teeth.

That threshold is only safe because the streamed multi-chunk path is no longer
covered by chance. It depended on whether the test server split a body across
frames, which left real coverage sitting just under 100 on some runs and would
have made the gate flake.
