Restore the _remove_duplicate_frequencies tests

The tests had been commented out and left behind when the suite moved to
pytest. Port them from the unittest setUp style to fixtures, matching the
rest of the file.

Both pass unchanged, so the original expectations were correct. Branch
coverage of applyaf.py rises from 92% to 94%, since the keep_max=False
path had not been exercised before.
