Commit Message

- Usage-related improvements.

- We now allow logging when no default adapter-name has been configured. This will allow a library to use logging when the application hasn't configured it or uses a different logging solution.
- We eliminated the notion of factories ("makers"). Whatever the original purpose of these, its useful was short-lived. Now we register using an instance directly.
- Improved documentation.
- Added console test.
