feat: add fetch-fun/openapi sub-entry grafting OpenAPI paths types onto the pipe

- createOpenapi<paths>() binds generated paths to
  typedUrl/typedPath/typedMethod/typedJsonBody/typedJson (~1 KB gzip
  with all dependencies, zero runtime of its own)
- typedPath: template must be a spec key verbatim; PlaceholderParams
  constrains the params object (missing/excess keys are compile errors),
  fillPath encodes values and throws TypeError at runtime as the second
  line of defense; the template literal type is kept as the phantom url
  so method/body/reader steps keep constraining against paths[U]
- JsonOk falls back 200 → 201 (RealWorld-style specs); JsonBody/JsonOk/
  PathKey/Op exported for user signatures
- dual-entry vite build shares config helpers in one chunk so both
  entries reference the same module-level symbols (readDataSymbol):
  self-contained per-entry bundles would duplicate them and break
  cross-entry pipes
- size-limit: index entry measures entry+chunk via esbuild now (6 KB
  limit preserves prior headroom); new openapi entry at 2 KB; attw moved
  to the node16 profile (subpath exports cannot resolve under node10;
  engines floor is 20.3)
- docs/openapi.md rewritten around the sub-entry; README.md and
  llms.txt references synced
