* Add message template to render

* Apply suggestions from code review

* Add aggregate-per-mention option to group messages by reviewer

When enabled, Pull Requests waiting for review from the same
mention are grouped under a single header line instead of one
row per PR. Also fixes a pre-existing bug in prettyMessage where
title/url were undefined and MS Teams messages lost their
markdown link formatting and line terminator.
