Fix Keymaster challenge response matching (#1263)

* fix(keymaster): match challenge credentials

Require each accepted presentation to satisfy one requested schema and issuer. This prevents schema-free or repeated presentations from making a challenge response appear complete.

* fix(keymaster): make response matching stable

Use maximum matching so overlapping issuer requirements do not depend on presentation order.
