Enable coverage CI workflow and set fail-under to 75% (#351)

The coverage.yml workflow was never enabled in this repository, so the
`coverage` tox environment was not being exercised in CI.  Bring it in
from qiskit-addon-utils, along with the Coveralls badge in the README.

Coverage currently sits at 77%, so the `fail_under` threshold of 100 was
unachievable.  Lower it to 75% in both tox.ini and pyproject.toml so the
workflow reflects the actual state of the test suite and guards against
regressions from here.
