docs: correct three stale counts and one stale format description (#73)

Three documentation errors introduced across the audit-logging series, two
of them raised by CodeRabbit on #72 after it merged and one found while
checking those.

AuditTracker's class docblock said "the first four name their new value" while
the list immediately above it enumerates five: position rotation, two
availability flags, two visibility toggles. Now "first five".

Both readmes opened with "Four values *are* recorded outright" and then a list
of the four service roles. That was true when there were four; it is now nine.
Rather than raise it to nine and wait for the next field to make it wrong
again, the count is gone: the text points at the per-field table added in #72,
which is the list that has to stay right anyway because it is the one that
enumerates every field.

readme.txt also still described the detail format as terse arrows - "a move
reads `Old -> New`" - which stopped being true in v1.31.0 when the strings went
back to full prose. README.md's copy of that sentence was updated then;
readme.txt phrased it differently and the edit missed it, so the file has been
describing a format the plugin does not write for two releases. It now matches
README.md.

No behaviour change; the strings themselves were right throughout.
