fix(darwin): give unauthorized and unsupported adapters their own errors

Every CBManagerState that was not poweredOn was reported as adapterOff, so a
denied Bluetooth permission and hardware with no radio were indistinguishable
from a radio the user had simply switched off.

The three need different things from the user — Control Centre, the settings
app, and nothing at all — and a caller had no way to tell which it was holding.
Map unauthorized to permissionDenied and unsupported to unsupported, at
startScan, connect, and the pending operations failed when the adapter goes away
mid-flight.
