Add once-per-code console footers pointing diagnostics at the repair skill

DEV.diagnostics.setConsoleFooter(fn) registers a footer printed after the
first console report of each diagnostic code (microtask-deferred so it lands
below the call site's own warn/error). solid-js registers it in dev to name
the repair skill it ships (node_modules/solid-js/skills/reactivity-
diagnostics/SKILL.md) — point-of-pain discovery: whoever hits the warning,
human or agent, learns where the prescribed fix lives with no prior
knowledge of the skill system. Registration lives in solid-js rather than
signals so standalone @solidjs/signals users never see a pointer to a
package they don't have.

Also records the bridge session-ownership constraint in the diagnostics
plan (global capture windows; multiplex or single-owner before any second
in-page consumer such as a devtools reactivity panel).
