docs: hand the AGT_SESSION_PWD contract to Session.cwd(for:)

PR #482 moved a custom command's working directory from effectiveCwd to
Session.cwd(for:) and left five places stating the old contract. Three of them
named AGTERM_SESSION_PWD, which nothing exports.

- Session.swift: effectiveCwd documents seeding only, and its seed list gains
  scratch, which makeScratchSurface has always used
- AppStore.swift: addSession's note says why its two inputs are sanitized
  instead of how the token resolves
- .claude/rules/menu-actions.md: same correction, real variable name
- plugins/agterm/skills/agterm/reference.md: matches the {AGT_PANE} wording and
  names the scratch exception
- SessionTests.swift: stale parenthetical removed, not corrected. The test name
  states the behavior and cwdForPaneResolvesPaneSpecificDirectory owns the
  resolver
