docs(changelog): one section per category in the 0.5.0 release

The section carried two '### Added', two '### Changed' and two '### Fixed'
headings. Content had accreted in batches under Unreleased over three months,
each batch prepended with its own headings, which nobody noticed while the
section was still open. Frozen as a release it reads as a mistake.

Each category is now a single heading. Content order is untouched within and
across the merged blocks, so the newest work still leads each list — only the
duplicate headings and the blank lines at the seams are gone. Verified by set
difference against the previous file: no content line added or removed, and all
20 '####' subsections still present.
