docs: record the MAPCO-11478 two-axis review

Replaces the feat/ogc-tiles review, whose findings were closed in 6012fb33 and
which remains in history at cd2f2488:review.md.

Carries what the branch cannot: which findings were fixed and where, which are
inherited from the cherry-picked commit and deliberately deferred, the two local
test failures that are arm64-only and predate this work, and one claim that was
checked and rejected.
