* fix: ignore captcha widgets on served pages

Shopify ships h-captcha-response in every storefront bootstrap, and
news comment forms call grecaptcha.render on 200s. Both were sending
every page up the proxy ladder.

* fix: match recaptcha endpoints only on google.com

(?:^|//) also fired when another host nested a google.com/recaptcha
URL in its path.
