chore: update DDEV TYPO3 multi-version add-on to 0.4.1

Removes the pre-marker utils.sh/index.php so the add-on's fixes for
dynamic ServerName resolution, the webserver_type freeze bug, and
classic-mode installs actually apply. Restores this project's own
TYPO3_VERSIONS=13 14 scope (composer.json only supports ^13.4 || ^14.0),
which the add-on's regenerated docker-compose.typo3-setup.yaml had
reset to include v12.
