fix(release): label changelog section with released version and fall back to unreleased notes

- pass --tag v$npm_package_version when generating the changelog during
  version bump so new entries land under '## vX.Y.Z' instead of
  'Upcoming version' (release notes lookup missed them, silently skipping
  GitHub release creation)
- release.js: fall back to the upcoming section if the tag section is absent
