Add `LayerNoiseModel` to `ExperimentalSimulatorOptions` (#3259)

### Summary
Updating `ExperimentalSimulatorOptions` to replace `noise_model` with
`layer_noise_model`, a sequence of `CircuitInstruction` and
`PauliLindbladMap` tuples.
