[fix] Fixed global notification setting integrity #503

Allowed only one global notification settings per user.
The IntegrityError is already handled by the code but
was never raised due to flawed schema design.
The new code raises IntegrityError if two different
processes create a global notification setting simultaneously.

Added data migrations to resolve any existing duplicates.

Fixes #503
