fix: report tool cluster availability, require usable profiles (#104)

* fix: count only readable, non-empty profiles as recorded

The glob also matched a directory named *.json and a zero-byte file, which
opened the profiler cluster with nothing in it to read. hasLogEntries() already
applied that rule; hasProfiles() now does too.

* fix: report tool clusters as availability rather than session registration

typo3-info runs in its own process and probes the state per call, while the MCP
session fixed its tool list at start. Reporting that as registered claimed the
session's registry: a cluster that appears mid-session now reads as available
with a hint that it arrives after a reconnect.
