* chore: update ddev-typo3-multi-version-extension add-on to 0.4.1

Removes the marker-less generated files identified in #41 so the add-on
can regenerate them, then re-applies the project-specific customizations
that the regeneration reset: TYPO3_VERSIONS restricted to 13/14 (no v12),
and the remote.typo3-file-sync hostname used by the fake remote server
routing.

* fix: exclude .ddev from PHP CS Fixer scan

The DDEV add-on regenerates .ddev/.setup/templates/index.php on every
update, so it isn't ours to format and will just drift again on the
next regeneration. It's the only PHP file under .ddev and had been
failing the CGL pipeline.
