Add a {nullable} doc tag for fields that may be JSON null

A pointer field whose null is a state of its own — "this value is absent", as
distinct from a zero — has no way to say so in the generated spec, so consumers
and generated clients type it as never-null. The tag is opt-in per field, so no
existing spec changes: only an annotated field is emitted.

docparse holds the OpenAPI 3 spelling and the openapi2 package moves it to the
x-nullable extension Swagger 2.0 uses for the same thing, throughout the schema
including nested properties and array items.
