Ignore `open` attribute on modal dialog (#169)

The `open` attribute is toggled client side by `show_modal/2` and
`hide_modal/2`, so mark it as ignored on mount to stop LiveView from
patching it back on the next diff.

Replicated to the installer template and covered by tests for the
closed, `open` and `show` cases.
