Portal profile menu dropdown so it layers above the AI assistant (#4646)

The `z-[100]` on the profile `Menu` wrapper created a stacking context
around both the trigger button and the dropdown, so the header trigger
also painted over the AI assistant modal (`z-40`).

Drop the z-index from the wrapper and portal the dropdown to the body
with the Headless UI `anchor` prop, keeping `z-[100]` on the items. The
dropdown still renders above the assistant modal, while the trigger
button now sits below it.
