Delete only revisions older than the one just uploaded (#309)

Two concurrent Put calls for the same user each kept their own upload and
deleted the other, so both could report success while leaving no avatar at
all. Cleanup now removes only the revisions uploaded before the file the
call itself stored, which leaves the newer of two racing uploads in place.
