fix: escape roff control characters at line start

escapeText escaped every '.' as \&., bloating output (e.g. "e.g."
became "e\&.g\&."), and did not escape a leading apostrophe, which is
also a roff control character. Guard '.' and '\'' only at the beginning
of a line, where they actually introduce macro invocations.
