docs: normalize changelog to the release-plz format (#776)

Normalize the hand-curated CHANGELOG.md to the format release-plz
generates, as part of the release-plz migration (see #772).

- drop the `## [Unreleased]` section (release-plz regenerates entries
from commit history; removes the need to maintain the `[Unreleased]:`
compare link each release)
- `*` bullets -> `-` bullets
- reference-style links -> inline links (version compare links, intro
links)
- delete the bottom reference block
- unify `### Breaking Changes` capitalization
