Merge pull request #64 — make §8b rule 2 implementable, stop the guard failing open

A CA bundle could not be supplied at all: both entry points dialled with Connection::connect, which takes no TLS material, so §8b rule 2 was not merely unimplemented but unimplementable. The scheme guard also skipped itself entirely when url::Url::parse failed. AMQP no longer routes through url_guard, so plaintext is refused on loopback too — REST and gRPC are untouched.
