feat: give Promises its own top-level menu, with Settings under it (#2)

The screen was registered with add_options_page(), so it sat inside WordPress'
Settings menu. An MCP server is something you administer — hand out a key,
revoke it, decide what it exposes — rather than a preference set once, and a
top-level entry is where a second screen would hang when one arrives.

registerMenu() now calls add_menu_page() and then add_submenu_page() against
the same slug, which renames the child WordPress auto-creates instead of
adding a second item pointing at the same screen. The menu reads
Promises → Settings.

The page slug is unchanged, but its URL is not: admin.php?page=promises rather
than options-general.php?page=promises. redirectBack() is updated to match —
the old URL 404s — and both readmes now say Promises → Settings.
