chat: a turn that boils down to nothing is not sent at all

A turn carrying only another provider's payload, or an empty string, went out
as empty content - the fallback internals.md warns against, because Claude and
Gemini reject it outright. An empty text block standing next to media or a call
is left out as well: measured against Anthropic, {"type":"text","text":""} is a
400, "text content blocks must be non-empty".
