* fix(cloudflare): detect JS challenge from HTML

CF managed challenges often omit cf-mitigated. The JSD
beacon also ships on 200 pages, so those HTML rules are
gated to 403/503.

* fix(cloudflare): drop JSD beacon HTML rules

The beacon is injected on ordinary 403/503 pages, not just
challenges. Keep _cf_chl_opt for the interstitial.
