Create table function data enumerator per scan init instead of per bind

DuckDB re-initializes a table function scan without re-binding when it
re-executes a pipeline, e.g. on every iteration of a recursive CTE. The
enumerator created at bind time was shared across all scans, so a re-scan
saw it exhausted and returned no rows, terminating recursion early.
