test: add consumer tests for rate-limited message visibility handling

Add 5 tests covering the new rateLimitedMessages handling in the consumer:
- Happy path: extendVisibility called with correct Math.ceil(remainingMs/1000)
- Ceiling: 1ms remainingMs correctly ceils to 1s
- Failure: extendVisibility rejection logged as warning, processing continues
- Mixed batch: rate-limited, valid, and invalid messages all handled correctly
- Rate-limited only: no entity fetcher or image processor calls
