Remove nested-scope config options

Drop `scope_nested_by_parent` and `scope_nested_through_controllers`;
parent auto-scoping and per-controller parent lookup are now always the
behavior. Override `get_recordset` for anything different — this drops
needless configuration for something with a simple escape hatch.

Also consolidate the association-serialization settings under one
comment and rename a local `controller` to `klass` in the native
serializer. Update the guide to match.
