[change] Resolved password expiration interference with SSO/REST API #511

Redesigned the password expiration handling to avoid conflicts with
passwordless authentication methods such as SAML, OAuth and sesame magic
links. Users authenticated without a local password are no longer asked
to update an expired local password.

Updated PasswordExpirationMiddleware so REST API clients receive a JSON
403 response with recovery URLs instead of an HTML redirect, which
breaks API clients.

Closes #511
