docs(license): separate dataset license from code license (#7)

The dataset in src/index.json comes from the Cloudflare Radar API, which
publishes under CC BY-NC 4.0. The repo-wide MIT license implied third
parties could use that data commercially, which Microlink cannot grant.

- NOTICE splits the two: MIT for code, CC BY-NC 4.0 for the data, with
  Cloudflare credited as source and the reformatting described
- README drops "official", adds a no-affiliation disclaimer and the
  non-commercial restriction
- LICENSE.md scopes the MIT grant to the software
- package.json declares MIT AND CC-BY-NC-4.0 and ships NOTICE on npm
