fix(mailer): send identity unlinked notification to pre-unlink email (#2692)

Unlinking an identity can promote another identity's email onto
`users.email`. When we send out an identity unlinked notification, we
should be sending out that notification to the previous address.

This PR focuses solely on the case where the Auth server handles the
delivery of the emails. The email send hook will be handled in a
separate, follow-up PR since it may require broader changes (e.g.:
exposing a new field `Recipient` to the hook).
