chore: tidy the PHPStan baseline (#45)

Regenerated from what is actually reported, rather than edited by hand.
Across the eight plugins that carry a baseline this drops 45 of 133
entries and 51 of 221 suppressed occurrences - all of them patterns that
had stopped matching, mostly "Else branch is unreachable", "never read,
only written" and stale "Used constant X not found" left behind by
earlier work. reportUnmatchedIgnoredErrors is false, so none of them were
failing anything; they were just noise claiming debt that no longer
existed. One amber entry also claimed 7 occurrences where 1 remained.

Regenerating also brings the file up to PHPStan'"'"'s current format: single
-quoted messages with less escaping, and an identifier: on each entry,
which makes them greppable by rule.

The ignoreErrors in phpstan.neon.dist are deliberately untouched, even
though some do not match locally. Whether those match depends on which
sibling plugins and stubs are present, which differs between a checkout
and CI - the config says so itself. Regenerating the baseline is safe
here because every sibling is on main, so what this run saw is what CI
checks out.
