ci: add workflow_dispatch and stop main runs sharing a concurrency group (#43)

Two problems the 2026-08-06 Actions outage exposed, both of which left
the release for #42 unreachable.

No manual trigger. A merge is the release, so when the merge's own run
does not survive there is no second way to cut one. The run for #42 ended
up wedged — queued with no runner, refusing cancel ("already completed")
and rerun ("already running"), and force-cancel with 409 "cannot cancel a
workflow re-run that has not yet queued". A merge cannot be repeated;
workflow_dispatch can, and it releases rather than just running the gates,
since a manual trigger that declined to publish would be useless for the
case it exists for.

One group for every run on main. A run that never starts blocks every run
behind it, which is what that wedged run would have done to anything
following it. PR runs still share a group per ref so superseded ones
cancel; main runs now get a group of their own. Releases still serialise
— that is the release job's own group, where it belongs. The gates never
needed to.
