refactor(rpc-handlers): extract activity-preview family behind facade [#857] (#2383)

* docs(architecture): add module decomposition program rubric + baseline [#857]

Establishes the recurring side-stream: extraction rubric (cohesion, churn,
coverage, regression risk — not file size), dependency-direction rules,
baseline inventory of large files, and the increment log. Anchors continuity
across future extraction PRs.

* refactor(rpc-handlers): extract activity-preview family behind facade [#857]

First pilot of the module-decomposition program. Moves the pure active-turn
activity-rendering functions (one-line collapse, tool/todo/question preview
formatters, row-to-entry mappers) out of the 4104-line live-query-handlers.ts
into a new leaf module rpc-handlers/activity-preview.ts.

Behavior-preserving: code moved verbatim, no logic changes. The public facade
is kept -- buildActiveTurnSummariesFromRows is re-exported from
live-query-handlers so existing dynamic-import tests and the NAMED_QUERY_REGISTRY
mapRow are untouched. The leaf formatters are module-private (narrow surface).

Adds activity-preview.test.ts: fast characterization coverage for the preview
branches that previously had only slow full-DB integration coverage. live-query-
handlers.ts drops from 4104 to 3760 lines.

* chore: retrigger CI (workflow run stuck in queued) [#857]
