Commit Message

Add threaded replies and reader mentions

- Add `parent_id` to comments to support one-level deep threads.
- Add `notify_on_mention` setting to readers.
- Implement mention detection and auto-linking in reflections.
- Send email notifications when a reader is mentioned.
- Update UI to allow replying and mention notification management.
