* fix: omit content-length when proxying streams

Upstream hosts often declare a wrong Content-Length while still sending
the full body. Forwarding that header truncates the proxied response.

* refactor: drop content-length from STREAM_ALLOWED_HEADERS

Single source of truth: proxy never forwards content-length because
streaming cannot verify it. Removes the redundant INVALIDATED_BY_STREAMING filter.

* docs: tighten proxy README section

Remove redundant prose, document decoded option, and clarify when
content-type is forwarded vs sniffed.
