docs: add Rector rule sets and the upgrade path from 1.5

One set per hop, plus a combined one. Only the 1.5 to 2.0 set rewrites
anything: two class renames and every container id. The other three are empty
and say why — 2.1 fixed return values, 2.2 only added, and 3.0 changed
configuration and schema rather than code. A set that looked like it had
handled the upgrade would be worse than one that admits it.

Removed services are renamed onto ids that do not exist, so the failure is
"no such service" at the line that wants one rather than a string that
quietly resolves elsewhere.

UPGRADE-RECTOR.md carries the checklists, which is where the work actually is.
