fix: use an explicit textEdit for auto-declare-namespace completions

eglot (and potentially other clients) falls back to inserting a completion
item's label verbatim at the cursor when insertText is empty, dumping the
whole `declare namespace tei = "...";` statement inline in addition to the
correct insertion in the prolog via additionalTextEdits. An explicit
zero-width textEdit forces the deterministic replace path instead.
