docs: document the ?? explain mode for annotations

The `??` trigger and the keyword openers that turn an annotation into a
question existed only in the agent-facing SKILL.md files, so a user reading
README, the site docs or references/usage.md had no way to learn about them.
The one user-facing mention described Pi's variant, which has no `??` rule and
answers in chat rather than reopening the explanation in revdiff.
