docs: encode one-cluster-one-room as a design decision, not a gap

Pulse is the single source of cluster composition: comms-gatekeeper maps
one cluster to one LiveKit room (island-{clusterId}) verbatim, and no
consumer shards or re-partitions clusters. Any future room/crowd size
bound will be implemented in Pulse at the tracker level. Reframe the
Known Architectural Issues entry, the runbook's percolation note and
LIVEKIT_ISLAND_SIZE row, and fix the openapi.yaml overview, which
wrongly claimed gatekeeper shards rooms.
