Fix lexer: triple-quoted strings confuse line/col tracking for later tokens (#394) (#396)

The triple-quoted string token regexes weren't marked lineBreaks: true,
so moo's own line/col counters silently stopped advancing across a
multi-line triple-quoted string's embedded newlines. Every subsequent
token — including the one a parse error eventually points at — then
reported a line/column from partway through the string instead of its
real position.

Fixes #394.
